This clinic is designed to assist individuals residing in California in diagnosis and management of various neurological disorders that are indicated below. These conditions should be chronic, lasting longer than 4 weeks. Commonly our patients are referred to us by a primary care provider for second opinions and or neurological evaluation.
We treat individuals over the age of 18. Please do not contact us for: 1) emergency matters, 2) refiling medications that are controlled substances, or for any specialty medications. We do not provide care for any urgent and emergent medical or neurological conditions. Please contact 911 or your primary care physicians in such cases.
Depending on proximity, at times we may need to schedule and see the patient in cinic in our Camarillo office. There are times we would need in person examination, selective neurological testing such as EMG/NCS and or EEG to be done. We will order labs and perform MRIs/CTs at facilities closest to your hometown for your convenience.
Pricing
We accept Medicare, medi-medi, PPO and cash
If you have any other PPO insurance, we can provide you a bill, which you can submit to your insurance company to get reimbursed. We recommend using Reimbursify.com to help you correctly submit a bill to your insurance company in order to get reimbursed on time.
Our cash prices are
- Office Consultation: $198.
- Telemedicine/Virtual Consultation: $198.
- Follow ups: $95 each.
- Forms to be filled out by physician: $5/page. Minimum $5.
- Prescriptions, MRI/CT or lab orders without formal clinic visit or evaluation: $50.
